Need Your Advice

My back to back IUI's are scheduled for tomorrow and Sunday but I still haven't surged. Yesterday I had 3 follies (biggest at 20mm smallest at around 16) and 1 on left ovary.

The dr. said he could give me an HCG shot tomorrow to bring on ovulation. Isn't it too early to be doing the IUI? I heard that sperm does not live as long when doing IUI.

Also, I'm on clomid this month and started on CD6. In the past I have started on CD3 and have surged on CD16 or CD17. My feeling is that because I started it a little late I will probably surge around CD18 or CD19. With an unmedicated cycle I usually surge around CD18-CD20.

Any suggestions would be greatly appreciated:)

TIA

Re: Need Your Advice

I am a little confused, why would they start you on clomid on cycle day 6 ??? I never heard of that protocol. Do you mean you started cd 5 ?? The two protocols are 3-7 or 5-9.

Your largest follie is 20mm....which is a good size but you have more time to see if the surge is coming on your own. Not sure why they want to give you the HCG shot, unless you don't O on your own...(You con't produce LH on your own). A follie , on the average...ruptures when it reaches 22mm. You could be surging today. I would go in today for bloodwork......and then go by that. However, a follie can grow up to about 26 mm and still be fine for fertilization. The nurse at RSofNY told me 30mm but I don't "buy that" lol. I am telling you, you really have to know what you are doing when dealing with all this.

IUI's at Re clinics are like....blood pressure checks at GP offices. They push these IUI's like there is no tomorrow. It's quite interesting.

If they give you a hcg trigger tomorrow,....not sure why they are also doing an IUI the same day. HCG triggers, act like LH and help rupture the follicule....it takes about 12-36 hours. Having an IUI done the day there is NO SURGE IS POINTLESS. IUI's have to be timed perfectly. The sperm does not live long while washed. The problem that, clinics are having lately, IMO is that they are not monitoring their patients well enough. Also, OPKS aren't working well lately....especially 'ANSWER BRAND'...(that opk is giving more false positives than ever).

If you get the hcg shot tomorrow, I can see Sunday's ....IUI a good time.

Why are they giving back to back IUI's ???? Is sperm count an issue ????

I know RSofNY doesn't like to do two IUI"s they rather you BD...if your CM is good.....

I believe you have a fertility monitor ??? (I forget)....that monitor is usually very reliable for surges.

Not sure if you understood what I posted, I think I rambled a bit,....but bottom line, if you aren't surging today,.....and not getting a hcg trigger today....I am not sure why, you are scheduled for an IUI tomorrow.

Re: Need Your Advice

Thanks for the feedback here.

I did start clomid on day 6 because I switched RE's and my appt happened to fall on day 6 and I really didn't want to lose a whole month so we tried the start on day 6. They said this was ok to do that you can actually start clomid up until day 7. It appears to have worked though because I have some nice size follies.

Anyway, I called the Dr. and asked to come in for blood work and they said it wasn't necessary that the dr. would do an ultra sound tomorrow to see if the follies have fallen yet. Plus they are telling me the sperm lives for 2-3 days after it's washed which I do not believe is true.

The only reason I can figure he's doing back to back IUI's is to "chase the egg" as he refers to it. I'm definitely feeling very crampy like something is going on today but my ovulation kit (the one I ordered on early pregnancy site) is not showing any surge yet.

Re: Need Your Advice

Just got my surge. I'm very excited:)

This is the first cycle I have tested 2x a day and I'm finding this interesting that I surged in the evening. This could explain why my IUI's haven't worked in the past...they might have been too late. For example, typically I would not OPK again until tomorrow morning and then I would have the IUI the next day. If the egg drops quickly after I surge and is only viable for 12-24 hours I could have been a day late. Does this make sense to you ladies?

Re: Need Your Advice

I just wanted to say GOOD LUCK! Glad you got the surge and lots of baby dust....

I also wanted to say my RE said that washed sperm could die as early as 12 hours after washing, and the max they have seen is 48 hours, so I would always use 48 hours as the utmost max as far as Im concerned, but theres always differnt opinions I find as well.

LOts and lots of luck!!!!!!!!!!!!!!!!!!!!

Re: Need Your Advice

Thanks:)

Let's just hope DH's swimmers live long enough to reach one of the eggs! I had my IUI's yesterday and she did a sono and saw there was only 1 follice left on my left ovary at 24mm and a smaller one on the right so I guess the other two on my left ovary fell. When I went in this morning for my 2nd IUI they saw all the eggs were gone with the exception of some smaller ones. I'm officially in 2WW. Here's to a quick 2 weeks:)
