NYC teachers-UFT dental claim form?

I had my wisdom teeth removed in February. I recieved a dental claim form and an explanation of benefits. The claim form is for Pre-treatment estimates and Claim for payment.

I already had the work done and so I'm thinking the claim for payment part is for me. But is it?

It depends..I get those, but my dentist just takes whatever the union gives him and doesn't expect any further payment...it might be one of those "this is not a bill" things you get, like from GHI?
