ARGH!!!
I was summoned to be an on-call Juror in Bklyn not even two months ago. So then WHYYYYYYY am I getting another summons to be an on-call Juror at the end of July in Queens!? Am I the only person in NY for them to summons!??? 
The system is complete BS!

did you serve 2 months ago? If so then you can just fill out the form telling them you already served and the dates. If you did not actually serve (meaning go to court and then either on a jury or were excused), then I think you are stuck.

Nope, I was on stand-by (I guess), because I never actually had to appear at the courts. I'm definitely stuck, because apparently Bklyn & Queens are different divisions, so I would have to be on standby now, too. However, I'm not necessarily stuck, because I have an excuse to not go... I'll be out of town. I'll be sending a copy of my airline ticket to be excused. 
I just can't believe that they've called for me AGAIN, in such short amount of time. BS, I tell ya!

I had to go a couple of years ago - I sat in the courtroom but they got their jury b/f I was called so I got to go home, but when I got another notice this year, I just sent them the copy that was stamped that said I had done it and that was it. Not sure how many years it is between when you can be recalled.
ETA - Oops - guess you responded before I hit post. Yeah, I guess then you probably can't be excused, but you can postpone I think.

Until you physically serve they keep calling you...its a pain in the arse but once you serve they don't bother you for awhile...

I guess that's right... I was called in in 2000 for a couple of days, and never was chosen... and haven't heard anything until this May... and again now... they must be making up for the past 5 years...
