Name Change Q
In my personal life, I am First Name Married Name.
But in the school world, I am first name maiden name.
Oddly enough, my certification is first name Married name.
Anyway, I am going to change my name to my married name with the BOE.
I have to let the UFT and insurance carriers know, too, right?

Yes there are HR forms you can fill out and fax- then they will update in computer for BOE and send you new ins. cards too- after u fax, you hand one of the papers to your payroll secretary - you can get the form online - here is the link http://schools.nyc.gov/NR/rdonlyres/79181142-4559-4B49-B3D3-BAB78FDE8CBD/0/PDC_Form_122309.pdf

Same name in school. I'm only changing it for the purpose of keeping all papers consistent.
