Well we take our 5 year old and our newborn to A to Z Pediatrics in Forest Hills queens, 718-575-2200 Dr Maya Golbraykh was recommended to us by a neighbor. We are very happy, Office is nice clean and its very convenient, 100 feet from the 71st Continental ave train station. Its not Long Island but I saw your post and wanted to reply.
