How does monitoring at SIRM work for IVF cycle?
I live on LI and work in eastern Nassau. Dr T is my last resort & biggest hope. If we decide on IVF, could someone explain an average cycle in terms of monitoring? Is there a lab or satellite office on LI for blood/sonos or is it all done in NYC? I will do whatever it takes but I'm a little
nervous about hopping a train for as many times I remember having to be monitored by RSof NY when I went there.

I'm in the same boat as you. I live in Suffolk and will by cycling in October at Sirm with Dr. Wang. I will be going into the city but Dr. Wang mentioned that I could monitor at another office (LIIVF, RSNY,etc). You would have to call the different offices and see who would allow you to monitor with them. Dr. Wang said he would be making all of the decisions and they would strictly be doing the sonograms and blood work. The only issue is that I think the other office would bill you (or insurance) for the monitoring. We are paying OOP so we can't afford the additional expense. This is my first IVF cycle with Sirm so I don't know much about their monitoring, but Dr. Wang mentioned he will need to see me 3-4 times before the retrieval. Other women on this site have mentioned that Dr. Wang takes appointments from 8-4 and Dr. T. takes appointments from 9-5. Good luck!

I live in eastern Nassau too. I just started with Dr. T on an injectable / IUI cycle. It's a pain, but just remember that it's really only about 2 weeks of inconvenience. I had my day 3 last Monday, and I go back tomorrow for my next monitoring. I haven't had a problem getting a 4:30 / 4:45pm appointment. And they run on time - it's amazing. I used to wait an hour+ at my old practice on busy mornings for monitoring.
If anyone knows/finds a practice that will work with SIRM docs to do blood and sono on LI, let me know. If this doesn't work, we will probably do IVF in a few months.

Thanks so much. I live in western Suffolk. I wonder how it would work to have another practice monitor you but not actually take you as a patient... That's an interesting idea. I would rather Dr T do my sonos but that's good to know...

And 5 minutes after I write this, Dr. T's office calls to say that my monitoring appt tomorrow is at 3:45. I said that's too early and I asked for the last appt and she said all the Docs are in a 4 pm meeting that came up last minute.
Great, now I have to stress all day tomorrow about making it on time.

This will be me too. Im going have to make it work if this is my last resort.
I don't know how I feel about monitoring somewhere else. Feel if I go with dr T I'd like him to do sonograms and bloodwork so there's no discrepancy.

Thanks so much. I live in western Suffolk. I wonder how it would work to have another practice monitor you but not actually take you as a patient... That's an interesting idea. I would rather Dr T do my sonos but that's good to know...
I didn't get anymore info about the outside monitoring because I figured I switched practices for the individualized attention and the level of care they provide. Although two hours of commuting is no picnic, I would prefer to have 15 with my doctor to discuss the progress of the cycle vs. a nurse or sonotech.

I went to SIRM for a cycle in March/April. I live in eastern Nassau and work in eastern Queens. I got out of work at 3:30 and headed straight to SIRM. Sometimes I would get a ride with someone from work heading into the city and it would take about 45 minutes to an hour depending on traffic. If I couldn't get a ride I would take the LIRR and walk from Penn- its about a 10 minute walk. I would always ask for the latest appointment and they would usually say that is 4. However when I started with them I asked Dr. T if it would be okay to come by 4:30 and he said yes he would wait for me. So when I scheduled they would write me in the book as 4:30 or 4:45 but they would always say we will call you to see if that time works. He does new patient consults at 4 and 4:30 I think so I would have to wait sometimes. I think twice they told me he couldn't do my later appointment because he had a meeting so I went in the next day and another time I said I have no other options and they accommodated me. You can get BETA BW done at Quest or your local lab.
It was very stressful trying to get there on time since my job isn't flexible but I had no choice bc I knew this was my last hope. Also I had to do intralipds so those appointments are earlier so I had to take off those days because I would never make it by 3.
In my opinion it is worth it! Hopefully its only a month out of your life of annoying appointments and stressing about getting there. ANy questions, just ask.

Glad I could help. If you get out by 3 and there is a train leaving around then you could probably make it by 4:30. A lot of times I said 4:30 and when they called to confirm they would say 4:45 bc he would be doing consults. Sometimes he would see me quickly other times I wouldn't be seen until after 5. There are a lot of trains heading back to LI at that time so you won't have to wait at Penn long.
I was given a typed calendar at the beginning of the cycle so that was helpful to go by as an idea of when things would be. I went in for the baseline and then maybe 5 times during the cycle. I'm a slow stimmer so I had to go back more than he wanted.
As for intralipids I used that and prednisone bc of an issue found with my immune testing. I believe using intralipids and prednisone is what did the trick. I have 2 failed prior IVFS and a failed FET. So I think using those 2 helped. So yes I was successful. I am finally preg and starting my 3rd trimester soon. They do intralipids later in the day bc they do them when all the IVF transfers are done. I think they usually have 2, 2:30, and 3:00 appointments. It takes about an hour to an hour and a half. I did it before my ER- I have to look exactly when and then after my BFP and until 16 weeks preg. It was once every 3 weeks. It was a pain but worth it in my opinion.
I also used HGH this cycle for the first time so thats something you might wanna ask him about. Also ask about intralipids and Prednisone. I also used Lupron at the start of the cycle and had never used that before. Those 4 things are really the only things I did differently. A new lab may have helped too.

Congratulations! I'm so happy for you that this protocol worked for you. I will make sure to keep all of these things in mind.
DH and I had some HLA matches and I think I may be a candidate for the intralipids too. I just wish I could get an appt for a consult before mid Nov.

Babysat dreams- I know this is an old post but I am about to start ivf at sirm and I have to do the Interlipids. I see that u were successful there. I'm so nervous. Any advice would be great
