i was in the same situation. i was thinking of how to put in a complaint professionally. i decided to go to my supervisors boss. just be careful not to totally bash your supervisor, i was afraid i was going to let loose and do that. i was very professional when i spoke to her and got my point out without beating around the bush. in the end, everything worked out.
ETA: i had to report her because she was giving me a hard time taking off due to my husbands illness. i work i the medical field and was absolutely appauled by some of her remarks

I am a supervisor and if anyone on my team had an issue with me that we couldnt resolve, they would go to my boss who is the director of operations.
I would send an email to the Supervisor's boss and ask them when is a good time for them as you would like to see them.
After a mutual time is agreed upon, I would tell them how the Supervisor is treating you and how you noticed it's not an isolated incident.
You dont want to bash the supervisor but want to express how her actions are affecting your work and how it's effecting the overall morale of the department.
