I mailed mine in ahead of time also. I think it made a difference with checking in b/c we were upstairs in a room in about 5 minutes when I was in labor.

I delivered at NS Manhasset and I remember getting the info at my Ob at the 28th wek. They told me I needed to pre register so I did as soon as I could and sent it in. i made copies of everything and about a month later I called admitting to make sure they received my info and it was in the system..

I mailed mine in when my GYN told me to and I made copies in case they didn't have it and I brought a copy with me to the hospital in my bag

mail it and bring copies.
I mailed mine in December sometime. I went into Labor Jan 31st and they said they never got it. DH had to give all the info because I was in the delivery room. Luckily I had the photocopies for him to work with because I am sure Dh would have struggled with some answers (like my ssn  ).

I never got to send mine in because I went early plus I ended up at Stony Brook instead of St Charles.

I filled everything out when I got to the hospital for my c-section. I never knew I could do it ahead of time.

I never filled mine out...
I had a sched. c-sect 2 weeks before my due date... and only was give a days notice that my dr. was doing so...
(I actually think I got the paperwork mailed to my house after I had Ryan and was still in the hospital.)
When I got there- I verbally gave them all my info.- Fast forward to 9 months later...
They tried to bill the last ins. comp I had (the last time I was in there as a kid...which obviously isnt the same carrier I have now) and my mom got a statement saying they rejected it....
God knows if they ever figured out who my ins. comp. was and billed them... but Ive heard nothing from no one!
