Passport
I 've been married for three years but never did a name change. I got it when I was 18 so it is still current but I need to finally change my name on it. Does anyone know what forms I need to fill out? Do I include new pictures? Is there a fee?
I got around to doing everything else right after the wedding but with moving around so much I forgot to do my passport and now I need to get it done in case I ever need if my husband gets injured.
Also do I need one for my son? In a worse case scenrio like mentioned above he most likely wouldn't be going with me but I also want to be prepared in case I ever get the opportunity to meet Dh half way someplace.

I believe you have to send in your original passport and the original copy of your marriage cert. Unless they've changed the rules, your son will need a passport too.

There is no fee if: Your name has been legally changed due to marriage or a court order within one year from the date of issuance of passport

Over a year, you have to pay $67 and apply as a renewal:
