you can get them from the Post office.
just downloand the forms, fill them out and hand/send them in with your check and those little photos
passports

We got our photos taken at Eckerd and submitted the forms and pictures to the Smithtown Post Office.

I went to the Town of Hempstead office. Was real quick. They will even take the picture if you need a picture.
I also got my marriage licence there. Quick and easy.

You can print out the passport application from home so you have it filled out. It'll tell you what to bring but I think it was 1 or 2 of those passport photos (I got mine done at kinkos), license and a check for the fee.

we went to the post office in the roosevelt field mall its downstairs outside of macys so easy and simple.
you need ORIGINAL copies of birth certificate they Will NOT take a copy, you get it back with your passport. and a drivers license.
I love going to the mall post office NO ONE is in there!!! in and out!
