Problems with Jury Duty Questionnaire!!!

I got two jury duty questionnaires in the mail - one in my maiden name, one in my married name...they give you a number to call if you have questions - obviously I want to tell them that I don't use my maiden name, legally, I use my married name and to "nullify" my maiden name.
No one ever answers the dang phone number!!!

Anyone else have this problem?

I don't want to go to jail for not filling it out! LOL!

Maybe mail both in together with a copy of your marriage certificate and a letter explaining that you use your married name?

I had to send in Sarah's birth certificate and a letter explaining that I have no backup daycare when I got a jury questionnnaire a few months back.
