Re: Anyone ever had an error on child's birth certificate?
There were errors on both Sarah's and Andy's. I was born out of state and for some reason they put different cities on both, neither one the right one.
I sent the town clerk where it was issued a letter and a copy of the birth certificate with the error noted, and then they sent me a form all filled out, I just had to sign it and send it back with the originals.

Yes - they spelled DH's middle name wrong on the parent part. I called the town clerk for a new one. Kept the old so we can confuse everyone later.

Re: Anyone ever had an error on child's birth certificate?
Jiavana's name was spelled with a "y" instead of a "v".
I had to write a letter with the correction and send the certificate back to good sam so they could fix. What a pain though. Because her social security card was wrong too.
So then I had to go to Social security, fill out a form, prove her name was spelled wrong (insurance card). And of course it wasn't a quick trip. Just like DMV...waited about an hour!!
