Going to DR tomorrow 1/17- update.

Ladies,

I'm going to the Dr tomorrow- my regular ob/gyn and I'd like to put together a list of questions/statements, etc. This Dr has the habit of steam-rolling me so I want to be prepared.

A little on me:

We've been trying since May. My cycles are all over the place- the short of 36 days the latest long one was this past cycle of 59 days.

I alternate between ovulating and non ovulating- there is no rhyme or reason. Since May I can say for sure I have ovulated two times. In June, and this past month I O'd on CD 46!

The last time I saw the Doctor was in October. She said she wanted me to wait it out until January and I made tomorrow's appt. She also wanted me to get a progesterone test- but I hadn't ovulated until end of December- and I didn't confirm this with an OPK- just major cramps, temp jump and getting AF 12 days later. I figured I'd just wait until my appt since I was travelling, etc.

So- any helpful questions/hints, etc of what I should be asking? I am posting over here because I really think I'm having major problems and will need to be on some sort of drug. I also have a sneaking suspicion that I am insulin resistant.

Update:

Ok- so here's the update.

Since I'm on CD 5 she is putting me on 50mg of Clomid for the next 5 days.
I am to use the ovulation kits and BD for 3 days once I get a positive ovualtion.

If I do not ovulate, but I get my period, I am to take 100mg for the next cycle.

If I do not ovulate, and I do not get my period, then she gave me a script for Provera to bring it on.

When I do ovulate I am to get my progesterone checked.

She checked my records and my thyroid function (all three- I made sure) has been tested and is good.

I go tomorrow AM for my IGTT (insulin glucose test).

She just figured since I was on CD5 to start with the clomid and depending what goes on with my IGTT to figure from there.

There is a RE on staff there as well that I will see if this doesn't work.

So at least it's a step in the right direction.

My advice would be to ask to have a whole bunch of bloodtests drawn. Most of which are included in an annual physical. Tsh, t3, t4 (thyroid), I would ask to have the fasting IGTT test (insulin glucose tolerance test) in addition to the glucose test and prolactin.

Personally I would not take any meds until my results came back and I had a better idea what I was dealing with.

Although your cycles are irregular, your dh should also have a s/a. Many times it's a combination of both people.

Ob/gyns are not trained very well in fertility , even ones that claim to be fertility specialists.
If you are insulin resistant, metformin would be a med to use and you may be able to avoid clomid altogether.

On day 3 of your cycle....you should have your fsh, lh, and estradoil checked. These are very very important hormones and again, they can help determine what type of treatment you should need if any. I would also ask to have a vaginal u/s done...so that the ovaries can be explored as well as the uterine cavity/lining.

I'm glad things went well for you. Just some words of advise, you could surge and not have it show up on the OPKs. You should be monitored via bloodwork and U/S for your ovulation. Also, don't take Provera until you have bloodwork to confirm you aren't pregnant. Lots of women don't get a "+" on their pregnancy tests until well into their pregnancies.

My dr. warned me about the whole provera situation with the pregnancy- thankfully they are close and have a blood lab on site.

I agree with Donna, the only way you will know if you ovulated, is if your chart shows it and/or you go for a progesterone test...7 days past the day you feel you O'd. In fact you should definitely go for that test...because, if your progesterone indicates you ovulated but is low, you will need suppositories.

I agree with Donna, the only way you will know if you ovulated, is if your chart shows it and/or you go for a progesterone test...7 days past the day you feel you O'd. I am already scheduled for Progesterone tests 7 days after O.. maybe I forgot to write it- I already have my script for that too.
