I have the DB-450 which is the general one to fill out after you are off the job due disability (in this case maternity leave). I can't file before hand. Is there any special form you need to fill out for pregnancy?

Once you fill out this form do you send a copy to the disability benefits bureau and one to your boss? It says to send the form to your boss but do you also have to send it to the disability bureau as well or does your boss do that? I am so confused with this?
