Disability Claim...
Who do I call to apply for disability once i'm on maternity leave and when do I have to apply...I need a phone number if anyone has it. I would appreciate it ... thanks...

My HR person gave me a form and I fill out part of it then my Dr fills out the other part right after I have the baby and I mail it back to my office and they take care of the rest.

I called and they said that I cannot pre-apply right now. I have to get closer to my due date when my doctor says I have to stop working. They are going to send me a form but does every company have their own form to fill out? I don't think mine does...

Double check with your HR, mine had one for the specific insurance company they use. It was the standard NYS form, but it had the insurance company's address on it.

Oh, I guess once you go out or have the baby send in the form the state sends you and then if there's a problem you can ask your company to get involved.

They said that my doctor has to fill a portion of it out and then my company has to fill it out too...

The one I used had a section for me to fill out and then one for my doctor. I think in my section I had to put stuff like where I worked, my title, my start date, etc. I'll try to look back through my paperwork this weekend and see what your company might need to fill out.
