Our one month old is using a size 3. Just do a trial and error as the doctor told us. If they gag and choke a lot then use a lower number. So far thank God DS has not gagged and choked...

I believe at 4 months I was using a Stage 2 Avent nipple. If your baby is taking too long to feed and falling asleep midway through, move up a stage. If the baby is gagging, choking or letting all the formula run out of his/her mouth...move down a stage.
