calling all the IF experts/researchers/scientists
I am going to see my PCP tomorrow. I figured since it has been 2 years since my last "I am TTC physical" I am due for another. Besides thyroid functioning, what should I ask him to test. He is awesome and will order anything I tell him too. My RE kinda poo pooed my immune issues questions last cycle so I was hoping if I knew what to ask for I could ask my PCP to write the order, then bring the results to the RE on the 23rd.
Any suggestions?

If the pcp can write you a RX for the RPL panel, ......(recurrent pregnancy loss...which is the basic immune panel) that is a good starting point. Many of these factors can be the cause of implantation/ivf failure.
What they will test for on this panel is
Antiphospholipid Antibodies Cardiolipin Antibody Anti Nuclear Antibody Protein C Activity Protein S Activity Antithrombin III Activity Prothrombin Gene Mutation Factor V Leiden Mutation Homocysteine PTT Lupus Anticoagulant Fasting Glucose/Insulin Tsh, TFT's and thyroid antibodies
Believe it or not that that's a basic immunological panel....(involving about 14 vials of blood). However, if you come back positive for any of those things, I highly suggest you go for further testing with the Millenova Lab in Chicago.

I just FM'ed you, but will state again. Ask him to write a script for a thrombophilia panel and and natural killer cell assay. Quest does this...

My insurance covered the basic RPL panel (GHI).
The NK assay and the other things I had done at Millenova was self pay....that came out to be over 1k.

It's actually a pregnancy loss panel, so perhaps they will ?
Actually if you go to a rheumatologist they can test you for all of this as well...a lot of these things have to do with auto immune issues.

my union covered evything up to diagnosis. Every now and then for some reason they pay for some things. I have no idea anymore.
Michelle- Do you think these things are still important to test for if you have not had pregnancy loss.

Without a doubt...it can be the cause of the ivf failure. Unfortunately, the conventional RE's in the US don't really give it much attention these days.
I have to be honest Pauline, if you are going to continue with NSUH they will not really be that interested in the results.
It's a hugely controversial issue in the world of fertility right now....
Tomorrow I meet with Dr. Chu and I am bringing along my treatment protocol Dr. Scher wants me on....I am not sure it's going to go over very well with her .

I absolutely agree. RPL is basically the same as implantation failure.

Just wanted to post that my sister had MAJOR GYN issues over the past 5 years. She was not TTC, not even in a relationship but after spending years at the same GYN who said he ordered "every" test to figure out why she was having long-term vaginally bleeding and other problems and putting her on all sorts of crap, her regular doctor had her immune system testing. She had 2 counteracting blood disorders and having them could actually prevent her from maintaining a pregnancy. Now she is doing SOOO much better because she is off the meds that were actually hurting her. If she had been TTC, she might never have had never known these issues without the immune system tests.

I was thinking of bringing this up as well to NSUH...but I wonder if you havent even mentioned it to Dr. Chu yet, then how can you say that they wont be receptive to the results?
Just trying to prepare myself for what they might say in return to me on my suggestions towards further testing...

Oh I most definitely brought it up to Dr. Chu on my first consult with her. It was the first page I had her read. lol I had the results for the RPL panel with me. I inquired about the different treatment for these issues with her.
I have gone for much further testing and Dr. Scher has a whole treatment protocol set up for me.
Some of the treatment for immune issues are accepted by RE's...for instance, folgard, baby asprin, and even heparin/lovenox shots on occasion. Using prednisone and IVIg...are more controversial.
I will let you know what she says today.

Yep and if you are positive for ANA , according to Dr. Scher's protocol, you take it starting cd 5....and continue, then when pregnant he increases the dose.
