Anyone work with a home health care agency and can help me with a problem?

I ask because I have an issue with the agency that is providing the home health care to my father. Unknown to us, he would have been able to go from the hospital to a nursing facility upon discharge. We were never counseled on option via the hospital social worker. I was speaking with a friend who is a social worker for a nursing home and she said that he automatically qualifies for nursing home care because of the feeding tube and to call the home health care agency's social worker and request a PRI be done. We did that and it should have been done this past week and they claim is was done and he doesn't qualify. The problem is that it was never done. His current nurse isn't able to complete the PRI so another nurse would have have to come to the home but no nurse ever did. I questioned the social worker and she said that maybe a previous nurse that knows my dad completed it and I told her that he has only seen his one nurse and she wasn't able to do it. she is going to check on it but I plan on speaking to the supervisor to get to the bottom of this.

The whole point of my post...does anyone know who I would file a formal complaint with?

Most likely the Department of Health as long as this agency is accredited. Is this home care agency on Long Island. I am VERY familiar with most agencies in Nassau and Suffolk. I am a social worker as well.

Sounds like a good old hospital dump job. Now everyone will be pointing fingers at one another. I work for a licensed home care agency, and have to order PRI/Screens all the time for my clients. It's usually a process, because you need to get a doctor's order for them. There are also only a handful of nurses who I guess are trained/licensed whatever who can do them. With us, we go out of agency usually, and it still takes awhile. I think the quickest I've gotten them done has been within 2 weeks after I get the orders from the doctor.

I think contacting DOH before talking to someone in the agency is a little extreme. I would try to get in touch with whoever is in charge of the department, and then their boss too. I know the person in this role at my agency is good at lighting fires under whoever to get things done.

My issue is that they are claiming it was done and told me he was not qualified for skilled care when in fact, the PRI was never done. That is my issue and I think it's quite a big issue when you are dealing with an eldery patient with critical care needs.

Again, I would call this morning and try to get to the bottom of things.

As I noted above, it does take some time to get them done. It's never been my experience that you can usually just send some one out. Could it be that they are going on an old one and waiting for a new one to be completed? Anytime there is a change, and especially if nursing home care is being recommended, a new one needs to be done.

I also have found that while someone may be eligible for a nursing home level of care, that does not mean that is what they get. I work for a waiver program and ALL of my people according to the PRI/Screen "are eligible". In NYS especially, if it is felt that someone's needs can be met in the home environment, that is usually tried first before sending to a nursing home.

With what I do, my people all want to be in the community (which is why it's called a waiver). If you want your father to be in a nursing home, you should probably plan a team meeting to discuss that, and the first person you should probably get in touch with then is his DOCTOR as well, because you will need their recommendation---and everything goes by their orders that is going on in the community.

Are you also his guardian or listed on the health care proxy to make decisions for him? This also can hold up the process for things to happen the way YOU may want them to. I am NOT trying to give you a hard time...........just being realistic about things I run into daily.

If things are bad too, I hate to say it, but maybe you need to have him re-hospitalized and make a big stink about discharging him home. But you need to be on top of everyone there as soon as he is admitted. Lastly, if you don't have the authority to make decisions for him, it can be hard to get healthcare providers (especially doctors!) to talk to you, let alone follow your wishes. Again, I am not sure if what I am telling you is what you want to hear, but I am just going on what you listed briefly about the situation.Chat Icon

P.S. Does he have a worker or nurse actually overseeing his case?

I know that where I work, we are contracted out to provide services from various places (i.e. certified home health agencies, DSS, etc.), and we can't do a lot of things if we don't get the orders or plan of care from them to do things. It's a messed up system, and certainly adds to the "passing the buck" of things.......

I would try to get in touch with that person too if this is the case.Chat Icon
