It's funny you posted this, because I actually get really offended when parents of students call me Ms. Rivera. I'm not a Ms!!!!! I don't know why it bothers me so much, but it really does!

Was Miss Maiden Name
Changed to Mrs Married name, but a few tests/worksheets have my old name and I could not white them out. It was hard to change!

Too many kids know me already and all my papers & files there were already in my maiden name. I found that for me, it would cause too much confusion & be too much of a hassle to use my married name there.
ETA: Although most of my students don't use my name... they just call all the female teachers "miss" and all the male teachers "mister"

I teach 6th and 8th graders. The 8th graders had me as their 6th grade teacher before I was married, so they call me Ms. Maiden Name and the 6th graders who are new to me know me as Mrs. Married Name...
BUT, to be honest many of the students just say, Miss. with no name at all.

Mrs. Married Name (or just married name or even a variation on it -- we can be a pretty informal school sometimes), but it took four years for my maiden name to cycle out. There were ALWAYS kids who insisted on calling me by my maiden name if they'd had me before I was married. My only rule was that if they'd only had me after I was married, then they had to call me by my married name. Eventually, it replaced the maiden one. (Gosh that was a long and confusing answer. Sorry.)

other times a combo of Mrs. married-name-maiden name
or miss. maiden name
some students just refuse to make the change to my married name
