changing name on passport
I have been married over a year and never changed my name on my passport. Does anyone know what form(s) I need to send in. I was reading the website but got confused! I have been married over a year so I know I will have to pay. The passport is also over a year old.
I do know that I will need a certified copy of the marriage license. Where do I get this? From city hall where I got it originally? DO I have to call in advance or do I just go there? Thanks for any help you can give. WE hope to go away in January!

You need to send your orginal marriage certificate in withyour payment. They send it back to you with your new passport. Mine took about 6 weeks.
