many offices will have a form for you to fill out and sign. if not, i would think you could write something like:
I, Ang, as the parent and legal guardian of lucas (full name and ss#), grant the current peds office permission to release his medical records from bithdate-present date to new peds office.
i would check with them to see if you need it notarized, but that's likely all the info they need.

The new doctor he is going to see should have a form for you to fill out. Usually you would fill it out at the first appointment but if they want the records before then, maybe they can mail it to you.

Our new ped had a form that we filled out, however, the peds office we left claimed they never got it so DH wrote a letter similar to what the pp poster wrote and that worked fine
