I have two godchildren. For both, I got a picture frame and then after the pictures were developed - a picture to go in it.
My cousin was just a godmother and actually got a ring with the child's birthstone in it! I thought that was nice.

You dont have to.
I never rec'd one when I was asked to be one.
I gave out small gifts to my DD's godparents.
