DOE- what to do after the baby is born (insurance)
I didn't go to the maternity workshop and probably should have, but what do I need to do in the hospital to get the baby under my insurance? Do I call GHI and let them know? Someone said to get a letter from the hospital and send to HR, but they didn't know the fax #... if anyone has some insight I would truly appreciate it! I don't want to sit on hold with HR connect lol

My OB wrote a letter and I faxed it to HR with his birth certificate and a form from the website. Call HR and they will walk you through it. You also need to enroll him with UFT.

I added him online myself when I got home from the hospital. I think my husband then faxed in the letter I got from the hospital that he was born. Once I got his birth certificate I sent that in.

You have to contact your HR dept to have the baby added. You'll have to send the insurance company proof of the baby being born - discharge papers worked fine. I think you have 10 days or so to add the baby after the birth. It's very easy.

not DOE but my husband works for the police department so we have the same insurance.. he was just recently in the office and they gave him the forms to fill out and I believe they can just be faxed in-- there are some things that need to be completed at the hospital I think (so he says- I haven't looked at the forms myself yet)

For a child of a teacher, here's what I would recommend: You can get a letter from the birth certificate office at the hospital - just ask them for one when they walk around to ask you about filling out the birth certificate form - and submit that to HR Connect. I recommend doing this because with my first child, I didn't know such a letter existed, and I was up at the town hall on day 29 (we had to submit it to DOE within 30 or 31 days of the birth) requesting a printed out copy of the birth certificate in person since we hadn't received one in the mail yet. Then you go online for the UFT Welfare fund and do a change of status entry online to add the dependent. I think you have to separately fax something to them as well.
