For Example, that shows when they ate last, slept last, were changed last?
If you wouldn't mind sending it to me as an attachment at [email protected] I'd greatly appreciate it. I am not too creative!

I don't have one, but a lot of daycares will provide you with this. You should see if yours has one, save yourself the trouble!!

Our daycare gave us ours. When DD was in the infant room, I would write in when her last feeding and diaper change were along with when she woke that day. Then they add to it as the day progresses so I know how she did with eating and diaper changes all day. They also write in any incidents, special things that were done that day (such as taking a buggy ride).
Now that she's moved up a class, we still get the sheets of her daily activities but I no longer write her wake up time, feeding and diaper changes.

I typed up a sheet with her likes, my likes, etc. when DD first started daycare. I added a pretty border to make it not so serious looking. I just outlined in there that I didn't want her in the swing or bouncer too long because of her flat head, that her bottles were drank cold, how she likes to sleep, diapering needs, etc. The actual schedule stuff...they typically provide a form for that.
