Does anyone have Caremark prescription plan through NYPD??

Does anyone have Caremark prescription plan through NYPD??
I am furious right now. DH has sent in the paperwork to the PBA not once but TWICE to get DS on our prescription plan. Of course today we go to pick up a prescription b/c DS has another ear infection and apparently they still do not have him on file  DH called Caremark and the PBA and neither have recieved any paperwork but yet the address DH sent the forms to was correct  I am on the phone with them now and I am ready to lose my mind.
did anyone else have this problem?
ETA: Just got off the phone with Caremark and apparently they never recieved anything from the PBA which is where we sent the form TWICE. Now I have to call the PBA

i do--we give the caremark card for all scripts at the pharmacy....what health plan do you have? we use GHI/Bluecross
ETA: wow that stinks! i have to say that when we got married i did the paperwork to add myself to his plan---and we only had to send in like 2 forms and fax something and that was it. done.
i wonder why its been so hard for you?

WE have CareMark through Empire BCBS. I really dislike them. And thier customer service s ucks.

well i got the forms direct from the PBA website
This is the enrollment form from PBA: Enrollment That has to be sent to 40 Fulton St.
Then DH brought home a form that he got from his delegate--it was a legal sized document---and we had to fill that out and send it to the address on the paper.
And I think I had to fax again the enrollment form one last time with a copy of our marriage certificate--and that was it. I was added automatically.
