Dr's note for medical leave from work
I have to have my dr. complete a form to put me on "disability" leave from work. I am sure they know what to write, but what are some of the things your dr. wrote if you went out before your due date.
I also have to submit a form that asks "How does your disability prevent you from working?" I know I cannot put b/c I am big, fat, and uncomfortable  . What did you write or what was your reasons?

That's what I wanted to know...if they get specific. As far as I am concerned, I am pregnant and a dr's authorization should be good enough without a detailed explanation such as swelling, etc.

I think your doctor has to be specific as possible. Some companies won't pay STD if you are just going out because you are pregnant and haven't delivered yet just because you are uncomfortable. I know this was an issue that came up on another board I go on. There must be a documented medical condition that prevents you from working like preeclampsia, abornomal bleeding, etc.

That's correct while my dr won't disclose the nature of the issue with my HR, for STD they have to be more specific. The reason is that STD is paid through insurance not your cie.
