For my kids first birthday I used Cuddly Characters. We were happy with them. You can also check out the Pennysaver or Yankee Trader, there are quite a few of those type of companies that advertise in there. Call around, the prices vary.

We had Small World Entertainment do the Sesame Street puppet show for Mallorys 1st birthday... It was very cute!

No I didn't.. they came highly recommended so I just stuck with them and didn't look around
ETA- we used a place called Party Kidz to rent an Easter Bunny for my sons birthday one year and my Mom used them to rent a Christmas Elf and we were very unhappy with them.
