I have it too! Thank god it does cover $50,000, but before you start IVF you have to notify the insurance company first. Good Luck

Re: Empire govt and ivf
I have to say that is the best coverage I have heard so far. I have oxford and I only have 10,000 for advanced infertility. IUI's 100% but I have $30 copays.

Re: Empire govt and ivf
We have this insurance as well. We feel really fortunate, because when I worked for Catholic Charities they covered ZERO.

I have it too and it is great! going to a center of excellence has helped because we haven't had any financial strain.

Re: Empire govt and ivf
Has anyone gone to East Coast IVF with Empire govt.? Can anyone explain why they assign an in-network and out of network provider to you?

Re: Empire govt and ivf
Both my husband and I have a 50,000 lifetime max for IF benefits.
Also if you use a center of excellence there is no copay (and I think co insurance but don't quote me on that).

i go to East coast not sure why they would assign an out of network when i first started going there i was paying 20% but they have a program there for financial hardship you need to send a letter to the patchogue office explaining that you are unable to pay the 20% due to financial hardship and they can waive the 20% and copays i no longer pay my $15 copay makes it easier on me that they just except the 80% the ins pays

Re: Empire govt and ivf
That's nice to know about the 20% and hardship letter. When I spoke to Maryann at the East Patchogue office, she told me that they did away with copays. I am calling her again tomorrow to have her explain everything to me again.

Re: Empire govt and ivf
So what are the centers of excellence in Nassau County? Also Dh has a scrotal ultrasound and an SA coming up this week. Do we need to notify for that? What about each and every doctor visit? Do we need to tell them about the RE, the urologist etc?
TIA.
