Honestly, for all of them, I would say Nordstrom. I have been buying eveyrthing from there and have not been disappointed yet. I go to nordstrom.com.
I also get my casual stuff from Old Navy.

regular clothes: online, Banana and Gap. Sometimes I catch a good sale at J.Crew
Workout clothes: Target
Other stores I love are H&M and Anthropologie.

I really can't name any specific stores since I tend to not be very loyal  I go through phases where I love a store for a while and then find somewhere else to shop.
