GHI PPO Coverage...

As per the RE request I called GHI to ask what is covered regarding infertility...

I was told mandated procedure are 100% covered and non-mandated procedures are covered 75%

These are the non-mandated procedures:

In-vitro Fertilization
(3 transfers per lifetime)
Gamet Interfallopian transfer
Zygote interfallopian tranfer
Frozen embryo transfer
anesthesia
culture fertilization
Preparation for embryo tranfer
Preservation of embryo

Is this normal coverage...is it good? Does that mean that IUI is covered?

Does it mean that the first steps with the RE are covered?

Thanks girls for any info that you can give me.

Honestly, you have to call back and ask them exact questions re: IUI, etc.
Every policy is different and has coverage in different ways. The only way to know for sure is to have an in-depth conversation with them.
I was on the phone for almost half an hour the day I called for info.

They told me to get procedure codes from the doctor and then call back....is that what you did?
