Centereach: Michael Anthony's - on middle country rd, across from McDonalds. (Not the one thats next to Nicholls)
Farmingville: Guadino's Brooklyn Pizza..shopping center on Portion Rd & S. Howell.

check out Stillridingpizza.com (many restaurants use this dough)
Oh yeah! La Cappanina by Island 16 has a whole italian menu...
As does Buona Sera in Smithtown

It is not on your list of towns, but try Hunki's in Plainview. It is a kosher place, so you can't get meat on your pizza, but the GF crust there is great.

I am not familiar with any pizza places in the towns you listed, but I know Saviano's in Bohemia does.
