We have GHI and I think we have the rider as well. I will check with DH when I get home about what it means. I know the PBA pays back 10 bucks for each 15 spent on a copay for the GHI plan. DH has never had a problem, with them, and I've only been on for 8 months.
But the prescription plan that is run through Caremark doesn't cover BC, even when its prescribed for a medical reason. That alone makes me miss my old insurance w/ Oxford.

I also have GHI through the NYPD If you have an emergancy and the hospital is in the plan than you are covered...even if the DR. is not in the plan...because you will be billed by the hospital...although be careful of this because the hospital tried to bill GHI..but when you go to the hospital its supposed to go to Empire...the Hospital is supposed to know this because of the CPB code on the card.
I know that an ER visit is $50 co pay and if you are addmitted its $300
I'm not sure about the optional rider...but I can say this...they have already sent me a package to let me know that I am fully covered for my delivery in January...they sent the letter in July. When my son was born my old Ins. Co didn't send me anything until he was actually born

DH is NYPD and we use his GHI benefits. I have had no complaints.
He also became Det so its the DET assoc GHI which according to them is worse. Les coverage on certain things, Im not sure...we havent gotten to any problems yet. I just found out that Im PG and even the prenatals were covererd...when I hear most arent. Doc visits have been fine and I do know that you can receive all of your copayments back.
HTH..that is all I know.
