High-Risk OB?

Hi everyone,

I had my appt yesterday for bloodwork and sono. Babies are measuring at 7w5d (right on schedule) and heartbeats were 125bpm and 145bpm. Everything looked great :) Beta ws 127,089 and Progesterone 25.3.

Question for you all - since it is twins, concieved via IVF and I had a C-section last time around (and will have one this time around) - do you think I should really be seeing a high-risk OB? I remember my OB with my son considered me "high risk" (his term) but he is not considered a specialist. I was planning to ask my RE anyway but what do you all think?

Oh and another question: anyone go on prometrium suppositories after being discharged from your RE? I remember being on them until 12 weeks I think with my DS because I was concerned about my progesterone levels. I remember my P was 31 and after going off of the PIO it dropped to 19.

Thanks Chat Icon

ETA: I am also 35 (just turned 35).

Re: High-Risk OB?

ive found in my research that the answer varies. most MFM (high risk specialists) want you to be referred by your obgyn. there are a few out there who will take you on without a referral. talk to your obgyn about it and see what they think based on your needs.
i have ivf twins and a clotting disorder and im not considered high risk. there was one perinatologist who would've seen me but i didnt want to deliver at the hospital he was affiliated with.

High-Risk OB?

just food for thought - when i left my RE he said i was borderline high risk due to conceiving via IVF and being on blood thinners due to thrombo issues that played a role in prior losses. i saw an OB who was fairly lax (which i prefer) and he said i didn't have to see a high-risk Dr. but if there were any warning signs of anything weird at all he would refer me to a high-risk Dr. right away. i agreed. my pregnancy went well and i felt great until 32w when i was put on hospital bedrest with pre-eclampsia. that spiraled in HELLP syndrome and i delivered a preemie via emergency c-section at 33w. i also had placenta accreta which almost cost me my uterus and went undiagnosed until the time of delivery.

while i don't personally believe pre-e could have been avoided, i do often wonder whether or not the placenta issue could have been determined earlier had i seen a high-risk Dr, specifically one that looks at all aspects of the placenta. if it had, at least i would have been mentally and emotionally prepared for a c-section and the risks and blood loss. would knowing that info have helped me at all, or changed my situation at all? who knows... that's what makes it food for thought Chat Icon im not sure the outcome would've been much different.

High-Risk OB?

I had a fetal demise around 28 weeks several years back. They determined it was due to a clotting disorder where there was no blood flow to the placenta. We conceived again after and I could not find an ob that would take me because as soon as I said "fetal demise" during the consult they got scared so I finally found one who referred me to a MFM practice which handled my entire care and delivered me! Today these MFMs no longer delivers so I have to see a regular ob and see the MFMs for just a consult to determine if I need to be on blood thinners.
