I felt like a "bad" mommy when.....
Today my co-worker was taking pics of Mikayla and we had her sitting on a rock (low to the ground) and she fell and hit her lip. I was standing right there but could not react fast enough  
I know it is not a big deal but she has a scrape above her lip and I just feel sooo bad

Ohhhh 
How about this one - I was getting him from daycare... he was sitting up, and when he saw me he got so excited (like he always does) - so there he is, smiling and giggling at me while I'm crouched down in front of him, and I go to pick him up, and he pushes his head forward, rams his nose right into my hard knee!
He immediately does one of those cries where he keeps going and doesnt inhale for air yet to REALLY let out the howl!
I was so upset I was crying holding him - he was so happy to see me, and what do I do? Knee him in the face...

i tripped while carrying her into church on her christening day... we both fell onto the ground in the parking lot thank goodness her dress was so long, it cushioned her fall.. i got a little scraped up, but was just so happy she was okay.. but i cried the whole day after that...

I was putting Jacob in the car for daycare one morning and he bumped his head really hard on the edge of the door as I was trying to get him in his carseat. He was crying his eyes out and I felt absolutely horrible  When we got to daycare he had a bruise on his head and I had to explain to them what happened...I felt like a terrible, terrible mother

DS was not too sturdy while sitting up on his own. I moved for 2 seconds, he fell back and bumped his head on his ELMO toy

I put dd in her bouncy seat when she was about 8 months old when she got out of the bath and didn't strap her in and she fell out onto the bathroom tile.
When she was older and just learning to stand I was sitting close by holding my nephew and she fell and slammed her head on a marble table and needed stitches
