I have my post consult w Trivax tomorrow after my first failed IVF. Compiling questions to ask him... Thoughts?

I'm compiling a list of questions for Trivax tomorrow.
Do you have suggestions for any particular questions I should ask. I have a few but wanna see if I'm missing something.

Stimmed using follistim menopur and novarel trigger
Saline sono- all was good
Sperm analysis- all was good
Lining was good
Bloodwork was good- low on Vit D taking care of that now
Hormones seemed fine- never got a call there was an issue until post transfer. Was put on progesterone and estrogen but before that levels were ok.

Embryo quality was BB decent although cells were 6,4,4 when I researched and there should have been more cells.
For a 3 day transfer.

Dd 3 day transfer with all three embies that made it. Fertilization didn't seem to be an issue.

I never tried to get pregnant before until recently. I'm down an ovary and tube. Between my age, very low AMH and issue, they suggested IVF sooner then later. I tried a few months naturally on clomid- negative, tried an IUI, negative. Went right to IVF since my ins covered it right away due to a tubal issue.

Maybe ask if he recommends any further testing? At my Ivf consult I was told I needed generic testing and an endometrial biopsy as part of "standard" work up.

I would ask: Why do you think it didn't work?
What additional testing is needed?
Do you suggest doing a fresh or frozen transfer?
Why did they do a 3 day transfer as opposed to a 5 day?
If you do another fresh cycle how would the protocol be different?

P.S. I have low vitamin D also. I'm taking a supplement.

I would definitely ask for more information in regards to the embryo quality. Embryo quality on Day 3 is typically not rated the same as embryo quality on Day 5 - on day 3, they usually grade it on a simple scale as there are only 8 cells - the double letters (ie AA) are usually associated with 5 day embryos. I would ask HOW can we improve embryo quality? This was our major issue in the past.

What was your suppression protocol? What dosage of follistim/meno? Did they use lupron (agonist) or or straight antagonist (ganirelix/cetrotide to prevent ovulation)? Have they run your AMH? Is your FSH normal?

You knew going into this that you had AMA and one ovary, so your protocol should be tailored to a Dx of DOR. Was it? There are certain protocols specific to that--MDL lupron flare, estrogen priming, supplementing with HGH, etc. You need to ask him specifically how he is going to address the issue of stimulating your ovaries better and helping support egg quality.

You are consulting elsewhere? Are you strictly looking at Long Island or are you considering talking to some more serious authorities on treating DOR?
