In case of emergency health care letter?

DH and I are going away for a few days and will be leaving DS with his grandparents. My mom suggested that I write a letter to leave with her along with his insurance card so that she could make health care decisions for him in case of an emergency.

Any one have a sample of a letter like this or know where I could find one? I'm not sure how to word it.

i used to work in doctors office...heres what i would write...

To whom it may concern,
I'm writing this letter In the case an emergency should arise with DCs name. i, your name, give permission to your moms name, The childs grandmother to make all necessary healthcare decisions. I will be available via telephone if there are any questions or concerns at the following # (put cell phone # or where you can be reached here)Also find attached a copy of the childs health insurance card. (make a copy of both sides) please feel free to contact me at anytime.

your number

if you have time you should get this notarized so there are no if ands or buts about anything. if there are going to be 2 people watching DC make sure you include both their names just incase ones not home at the time.
