I have always just gotten a sweatsuit from the gap or old navy and then i bring it to a place to get embroided.
The places that sell it already made up are so much money.

even easier -
buy a sweatsuit from old navy or ny &co or something, and bring it to a local embroidery place.
I paid $7 for the sweatshirt (on clearance at NY&Co), and $15 for 2 color embroidery, with rhinestones.

There's an embroidery stand in the Roosvelt Field Mall. I believe it's by Wilson's Leather.
