Most IF coverage is determined by the employer, not the insurance companies.

Yup!
I am fortunate that I live in a state where IF coverage is mandated. HOWEVER, there are loop holes to this and not all compaines need to follow the mandate.
My company included. I'm lucky in that I had pretty good coverage, but it is NOT the coverage that is mandated by the state. When I questioned it, I was told it's about how the company "funds" the policy.

Since it is considered "elective" treatment, I don't think infertility benefits will be subjected to the no lifetime limit...not to metion the other restrictions that can be placed on elective treatment (age, other health conditions, etc).

yeah, I wanted to know what that change might have been as well. Not that it matters to me anymore (done with IVF's at this point).
Being a small business owner (50 or fewer employees), I cannot even get an IVF rider at this point offered to me. So we only have basic IF coverage. Which is included with our policy. (and luckily it includes a lot of stuff I didn't think would be included).
But, I'm sure whatever they do, they wont get it right for IF.
Maybe I'll search online later.
