Medical Authorization
Hi...my DH are going away and leaving our DS with my in-laws.
Does anyone have a form or the correct wording that would allow them to obtain medical treatment for DS...if needed?
Thanks.

Re: Medical Authorization
Here's one for nannies online. I imagine it's similar wording. If you're in East Northport, I can bring my notary stamp home if you want it notarized.http://www.4nannies.com/Forms/medicalrelease.pdf#search='medical%20treatment%20authorization'

Yes. It also indicates on the form that you should make sure your doctor will accept it.
