My job is looking to hire a MSW
Service Coordinator is needed for the home care agency I work for, you could work either in our Lynbrook office or Lower Manhattan.
Field work involved.
Clients have TBI and other complex medical needs.
I know my boss is looking to hire someone who would be willing to work in Brooklyn/Queens/LI but there is a chance the person would need to carry cases in all 3 places initially until your caseload grows.
It's not that bad if you plan out your days in advance and most clients are pretty easy to get in touch with (I carry cases in Nassau, Suffolk and Queens).
You have to visit the client at least once a month. Minimum caseload is 14 clients.
You also are not required to transport clients in your own vehicle but will probably need a car to make your life easier in seeing your clients.
You need to have at least 2 years of documented case management (not internship) experience with people with disabilities. Medical/mental health background is VERY helpful.
You also MUST be extremely resourceful and should feel comfortable working independently without a great deal of oversight and supervision.
Ideal candidate should have extensive knowledge of resources for the disabled in NYC/LI, as well as benefits/entitlements, and should be extremely organized, be able to think/plan ahead and absolutely must meet or exceed paperwork deadlines.
Helping clients locate housing is also a part of the job. You also may need to go down to DSS/HRA on occasion, and sometimes we do have to travel out of the immediate area to meet with clients we are bringing into the community from nursing homes.
