Need help with a medical record release letter
Can anyone help me create one? I have no idea what information I need to provide but I need to get my son's pediatrician to release his medical records to a new doctor.

it doesn't have to be anything fancy. i wanted to see my records from a doc i saw in college and just sent an email that said "Please release all of my medical records to me. My address is as follows...."
that's it!

FM me if you need a sample. I can fax or e-mail you the HIPPA compliant release form that I use to get my clients' records. You would just have to take out my office address and put your address in.

I just wrote one. All I included was my name, the name of the doctor I saw (it was a group), and a statement saying I would like my medical records released, in this case to my husband (he picked up the file). But if you're having them sent to a doctor, put the doctor's name there.
