One or two to transfer ?
CGH is out of the question .. I might only have 5 eggs. I'mhoping for some blastocysts so I cantransfer one. DH and I already have a 19 month so we fear twins and chromo issues because I am 40. We are dong the retrieval tomorrow then IVF/ICSI. IF we don't get to blasto am I really foolish at 40 to transfer one, embryo or blast? We are willing to do 3 or so more ... really don't want to. Any thoughts? I son't want to sound evil but if we get caught in the multiple situation has anyone known anyone who has done selective reduction. We do not want DS to be an only child but want to be as realistic as possible about our situation.

well can you afford to do more ivf if you transfer only one and it doesnt work? if you can afford to...then transfer only one.
if you cant really afford to...imo i would transfer two.
as far as selective reduction goes i think that is something that you have to talk about with your doc when you guys are decided to transfer one or two.

I was also afraid of multiples when I did IVF so we did a single embryo 5dt and fortunately got PG with my who is now 2. I was 34 at the time and my RE suggested that we transfer 2 embies but DH and I stuck to our guns and are glad we did. I am now 36 (almost 37) and this month I am doing a FET. Once again we will only transfer a single embryo.

I will do a single embryo transfer when we do our FET cycle next month. The last FET cycle (that we got PG with DS) we transferred 2 and I ended up PG with quadruplets The other embryo seemed to have split twice, creating identical triplets. They stopped growing early on, we never saw the heartbeats. So in your situation, I would transfer only one, especially because the chance of multiples is higher just from doing IVF, even from just one embryo.

If I were you I would just transfer one. We transferred two and knew the chance of twins and were ok with it. I am 19w pregnant with .
My RE discussed at length with us how many to xfer. They knew we were ok with twins which was the only reason they transferred two. They told us if we had any doubt to only xfer one.
We may go for one more. We'll see.. We would never transfer more than one again because we know we would not be alright with another set of twins.

P.S. I don't know about selective reduction for twins. In my doctor's office they only mention it for triplets or more. I think more than 3, they would definitely lean towards SR.

i would def transfer two - esp if they arent top quality. i transferred two perfect blasts and ended up with one baby and one empty sac. if we try again with our frosties we will likely transfer two again. more people who transfer two end up with one or no babies then end up with twins. statistics make sense to transfer two. good luck!

I completely agree with this. We have done transfers (fresh and frozen) with 2 or 3 embryos and have not gotten pregnant at all, so IMO I would never put in less than 2 UNLESS it was a hatching blast and I could afford to do more cycles if that one didn't work.

I was faced with the same question a few months ago and even posted it on here too. I have a 2 year old DS and was nervous about twins. After a lot of back and forth we finally decided to transfer 2 perfect embryos and neither of them took, then the next month the same thing happened, this past cycle we transferred one blast and one morula (stage just before blast) and I am 6 weeks pregnant with just one baby.
It's a hard decision to make. I pay for everything OOP I decided that I wanted the best chance for each cycle and I knew that even if we wound up with twins I would be OK with it in the end. If you are adamant about not wanting twins, then I would just transfer one.

The cancelled IVF goes against my ins. They still count it in my three and still have only two left covered- they treat is as a used up IVF.

Meaning statistically, ur more likely to not get pregnant or get preg with a singleton than having the 2 embryos taking, resulting in a twin (or more) pregnancy. HTH

Many people do a single blast transfer. I just had a patient a few months ago. 41 or 42 pregnant with a singleton after a single blast transfer. Keep in mind, this was her 3rd fresh IVF cycle. She failed 2 prior.
Selective Reduction I don't believe is done on identicals. Due to the risk of the 2nd baby being affected. So if you have an embryo that splits, that's an identical, vs 2 embryos taking and resulting in fraternals. Fraternal twins being reduced I've seen.

LOL yes this is what i meant. out of all the people who put back 2 - twins is still the least likely outcome - with BFN or singleton being the more likely outcomes.

Are you a nurse or Dr.? The 41/42 year old - all she did was single blast transfers then got lucky on the 3rd try?
