Outpatient clinics that specialize in substance abuse

I am fairly new to doing social work on Long Island (most of my career has been in NYC). What are some good outpatient clinics that specialize in substance abuse treatment? I am looking primarily for Nassau but would also take Suffolk resources as well.

Also SA places that would be willing to work with people who have some cognitive impairments as well as part of their diagnosis. Oh, all of the people I work with have Medicaid although some of them do have Medicare and on rare occasions, another insurance in addition to the Medicaid/Medicare.

Brookhaven hosp. "had" a program (not sure if they still do.
-mather hosp definitely has an outpatient

I know there is Pederson Krag as well as Seafield (which is an inpatient program in the Hamptons.. but they have outpatient places throughout the island)

I think these are more in Suffolk though.

If you just search the internet you should be able to find a bunch and about how their insurances work.

South Oaks is right by us in East massapequa (right by sunrise mall) I am pretty sure they have in and out patient programs

Catholic Charities, there is one in Commack but I believe there are other locations
