A part-time position of Case Manager is open in the Senior Services Department. This position is responsible for conducting assessments or reassessments, developing care plans, authorizing services, or terminating or discharging clients for Home Delivered Meals Program.

Minimum qualifications are: Graduation from a New York State or regionally accredited college or university with a Bachelors degree and two (2) years of experience in social work in a public or private agency. Additional education in social work may be substituted for experience on a year-for-year basis. Proficiency in Microsoft Office Professional a must. Valid drivers license required.

19.5 hours per week. $20.00 an hour.
