That's what I did for the RS and it wasn't covered...

It depends on your insurance...
We have empire (united healthcare) and I called the number on the back of the insurance card...they direct you from there.

We have GHI and I called Apria and all they needed was our insurance info and a prescription from the doctor.

First thing I did was call my insurance company to see if they covered it.
When I was told they did, I had to have my PED write on a prescription form what the problem was, and they faxed it over to my insurance.
Then my insuarnce sent it to one of the places they use.
