It's from Ikea. You can leave it on the bench part or you can mount it to the back of the stand.

i have a low bookcase for tv and 2 taller narrow bookcases on either side. bought at pottery barn about 3 yrs ago. they are discontinued now but i am sure you can find something similar.

We got this one from Walmart for $169 - its the Better Homes and Gardens Furniture Line (Willow Mountain Entertainment Credenza)
The color in the picture is REALLY off though, its a very dark wood color, more like the picture below (but not this stand):

We have this one - but our TV is larger and takes up the entire open space in the middle.
ETA: here is just the TV stand portion (in the lighter wood color to show how there's a shelf for DVD, cable box, etc. that you can keep open or close, etc).
And here is the stand with the hutch part (without the left and right cabinets that ours has)
