I think it is called Dix Hills Medical Associates

Carfora Family Medicine - there are 4 women doctors there and 1 physicians assistant. They are really great, take the time to listen, i never felt rushed there. They are in Commack on 347.

Dr. Marc Allen is awesome, all my friends use him so I drive all the way from West Islip to see him and his staff.
