The birthday would be underneath the name.

I wouldn't do the last name at all - unless it's part of monogram initials.
I'd do John David in that case

first and middle.
i got one that has a last name, and it seems a little weird. i also didn't like to take it out in public, because i didn't want to put our last name on display.
i got another one that had my daughter's birthdate wrong. it was the wrong year. based on the date on the blanket, i would have had my two daughters 6 weeks apart. lol. That one never came out of its wrapper.
