You need to be added in the first month after the wedding, or you have to wait until the open enrollment period (which I think is in October).
I can't remember where DH called to get the forms to fill out, but you will need to send them in, and definitely follow up with them to make sure they received it and are processing it. I have a friend who is furious right now because they messed it up when the forms were sent in. Luckily she is still covered under her own insurance because she didn't want to cancel until she was covered.
I'll ask DH where he called when he wakes up.

I think he sent the form in without the marriage certificate and followed up once we received it, but I am not sure. I think I remember them saying he had to have that before I could be fully enrolled, and then the benefits would be back dated to our wedding date.
There is a number on the second link I posted above for the City Health Benefits office.
BTW, thanks for asking this question - as I was browsing the PBA site for the benefits forms, I realized we may actually have vision benefits, that I don't think DH was aware of!
