Rehabilitation/Nursing Home recs?
My grandmother has been in and out of the hospital a lot this year because of her congestive heart failure. She lives in assisted living and the past 3 times she's been discharged from the hospital, I've had to hire a private 24/hr aide for a couple of weeks just til she gets back on her feet. That's been such a ridiculous amount of money. She's in the hospital now and finally, they're recommending that she goes to rehab for a couple of weeks this time, which I think will help her immensely. The social workers at the hospital want me to give them a list of 6 facilities that I would like her to try to get into. I live in East Northport, but work in Levittown, so anything in between would be great.
I know she's going to need to be in a nursing home within the year or two anyway, so this will be a good dress rehearsal for when that time comes, too. I did a little research already, but it's still hard to sift through reliable info and plain stats.
So far, I'm liking: Carillon in Huntington Huntington Village Rehab and Nursing Huntington Hills White Oaks in Woodbury
Any advice?

My grandparents both spent time in Broadlawn Manor, which is on the grounds of South Oaks in Amityville, right on Sunrise Highway. It was a nice facility from what I remember. This was over 8 years ago though.

My dad went here for rehab and then he is now in the nursing home part of the facility. They just remodeled the place and it came out nice.

Have you looked at St. Johnland in Kings Park?
My father was there for 3 months after a stroke back in 97 and my DH's aunt was there earlier this year.

Have you looked at St. Johnland in Kings Park?
My father was there for 3 months after a stroke back in 97 and my DH's aunt was there earlier this year.
My father was also a patient at St. Johnland for several months following his stroke about 5 years ago. We were extremely happy with the care he received there.
